I personally think that a major problem lies with the shops, on most occasions, as my wife and I carry foldable or cloth bags when buying things we have to tell the person serving that we do not want a bag. sometimes we have to raise our voice a second time, as the assistant is busy chatting to a colleague, and ignoring our "no bag required" request. as I see it, there are two answers to this problem, A-charge 10p per bag, B-encourage shop staff to ask if a bag is required. many shop assistants just put one small item in a large bag without thinking,
