The 33-year-old has struggled for form and fitness as Manuel Pellegrini's side scraped a top-four finish ahead of local rivals United this season.
And incoming boss Pep Guardiola, who sanctioned Toure's switch to City from Barcelona, is thought to be keen on moving the Ivory Coast international on as he remoulds his squad this summer.
He played a key role in securing their first silverware in 35 years when City won the FA Cup in 2011.
He has since added two Premier League titles, two League Cups and a Community Shield to his medal collection.
